363 TRICUSPID ANNULUS DISJUNCTION: THE MISSING PIECE IN THE ARRHYTHMIC MITRAL VALVE PROLAPSE?

چکیده
Abstract Background Arrhythmic mitral valve prolapse (MVP) is characterized by left ventricular (LV) fibrosis, due to an excessive myocardial stretch on the LV basal-mid inferolateral wall. Mitral annulus disjunction (MAD) and curling are common findings in arrhythmic MVP patients, thus representing traction of apparatus myocardium. Cardiac magnetic resonance (CMR) demonstrated clearly identify these typical morpho-functional alterations, as well patients. The involvement right-sided disease recently emerged interesting missing piece natural history However, its role a population patients absence significant regurgitation (MR) has not been evaluated. Purpose primary aim study was evaluate prevalence tricuspid (TAD) Secondly, we evaluated TAD severity alterations MVP, MAD curling. Methods Consecutive with normal systolic function without MR, referred for palpitations, were enrolled. Morpho-biventricular evaluations, including assessment, fibrosis using CMR. Results 66 met inclusion criteria (29% male, median age 44 years). identified 22 (27% 52 No differences arrhythmias (p: 0.459) between (n: 44) 22) identified. older (52 vs 42 years, p: 0.009) presented reduced biventricular end-diastolic volumes, despite normal, comparison those (left ventricle: 80.5 93.5 ml/m2, 0.002; right 72.5 85.5 0.018). more (respectively, 25 19, p:0.014 24 20, 0.002) than TAD. Conversely, severe 5.9 4.7 mm 4 3 mm, both 0.034) LGE distribution two groups 0.062). Conclusion In presence associated great So, uncommon, contextual suggests biannular disease, increased stretch. Conversely MAD, wasn't arrhythmias. Thus, arrhythmogenesis appeared limited.

Morphofunctional Abnormalities of Mitral Annulus and Arrhythmic Mitral Valve Prolapse
BACKGROUND Arrhythmic mitral valve prolapse (MVP) is characterized by myxomatous leaflets and left ventricular (LV) fibrosis of papillary muscles and inferobasal wall. We searched for morphofunctional abnormalities of the mitral valve that could explain a regional mechanical myocardial stretch. METHODS AND RESULTS Thirty-six (27 female patients; median age: 44 years) arrhythmic MVP patients w...
متن کاملThe unsaddled annulus: biomechanical culprit in mitral valve prolapse?
M asterful mitral valve (MV) repair demands thorough understanding of integrated mitral apparatus structure and mechanics. One consistently reported feature, the saddle shape of the mitral annulus, was recognized in the process of resolving the contradiction posed by an apparently high prevalence of MV prolapse (MVP) in otherwise healthy individuals: A normal saddle-like shape will show opposit...

Marcus-gunn phenomenon with- mitral valve prolapse
Marcus-gunn phenomenon is a congenital and sometimes hereditary anomaly, in which a ptotic eyelid retracts momentarily when the mouth is open, or the jaw is moved. These associated movements attributed to abnormal connections between the central mechanisms innervating the pterygoid and elevator muscles. Sometimes this anomaly is associated with dual,s retraction syndrome, duplex kindneys and ur...
